Q. What is the best way to reach out to potential clients?

I have had the good luck of securing a lot of work through recommendations and word of mouth but I would like to start approaching potential clients on my own, clients with a valuable mission that I feel could benefit from the video services I provide. I have tried to work out how to approach these companies... E-mail? Walk-in? Phone? And once I have their attention, how do I propose the idea of hiring me? Do I present a project proposal? I have a list of ideal clients and I feel strongly about what I can do for them, I just do not know exactly how to go about making a proposal. Any help is greatly appreciated!

Depending on what industry you are in. It looks like you are in the photography sector, which is an activity people usually do online nowadays. I'd recommend you starting with publicizing your website, You can do so by putting up ads on popular search engies, such as Google, Bing, etc. Meanwhile, keep updating your company's faceboo/twitter account! if you stir up some topics, people would hear about your company, would participate, and would spread the word! Topics can be something like "post your favorite V-Day picture/vedio", and even better, to win xx prize if your financails allow.

hope this helps.

You may want to partner up with existing marketing companies who can offer your service as a value addition to their already established client base.
